This is correct. 1 in 4096 chance for wilds and 1 in 216 for hatches. Nest Pokemon can not have perfect IVs though. Originally they could only have up to 3 Attack IVs. But, since then they've changed it to only limit it a stat to 9 rather than 3 and the limited stat is chosen at random.
Also, I caught one wild Eevee with perfect IVs and hatched 1 Goldeen with them.
The attack IV bug was related to pokedex number. Early on, wild caught pokemon were more likely to have an Atk IV proprtional to their pokedex number. Wild caught Bulbasaur, Charmander, and Squirtle would more likely have Atk IV around 0-3. On the other hand, wild caught Eevees, Snorlax, and Dratini would more likely have Atk IV around 12-15. The odds for a wild high pokedex number pokemon with 90%+ IVs were significantly better because of that bug. That bug has been fixed.
The poor nest IVs was also a bug and never intended. You can catch nest pokemon with good IVs now. In fact, if you are looking for a specific pokemon with good IVs, a nest is probably your best option just because you catch so many more in a shorter time.
I'm level 33 & don't have a single 100% Pokemon. Granted, for a long time, I didn't consistently check IV's. But, Id say from level 22-33, I have consistently checked them. I have multiple mons w/98% IV's-Pokie Genie will say, for example, 15/14/15, 98% IV. (Prob obvious but not sure why this wouldn't be 99% IV). And in my experience, I've had consistently better "luck" getting high IV mons in certain Pokemon. For example, I keep making 95-98% Exegcute's & Arcaine's but many many others, I can't get above 80-85% to save my life. So, my extremely long answer to your question is that it is very very rare to get 100% IV. (Unless I'm just cursed). That said, in my experience, most mons above 75% have no problem either attacking or defending gyms. I wouldn't hesitate to evolve a 90% Magnetite.
It's 98% instead of 99% because if you do the math and divide 44 by 45 the answer is 0.977...). ;-)
You have a 1 in 4096 chance (0.024%) of catching a perfect IV mon, but a 3 in 4096 = (0.073%) chance of catching a 98% mon, so it's a little bit more common. You also might just be getting lucky with your hatches/catches seeing as how you get so many high % compared to middling IVs. =D
I have five total; three were hatched, one was definitely caught wild, and I think the 5th was caught wild. Most recent was a wild-caught level 30 Horsea, which evolved into my second perfect Seadra. Both have dubious movesets. I also have a perfect Scyther (SW/BB), Exeggutor (C/P), and Charizard (WA/DC).
I am level 34, BTW.

I think it is totally random because all my 100% IV ones are from the last 2 months while on the first 4/5 months playing I did not get any 100% (unless I transfered it by mistake...)
I have at this moment 12500 mons caught, so the ratio for me is 3/12500 = 0.00024

I'm lucky that I have 20 perfect mons. Hatched over 1600 eggs and caught over 10k mons. I appraise every mon I catch. Before appraisal feature was added I used pgnexus. Now I use Poke Genie. I pretty much checked IVs very early in the game since July. IVs are extremely important to me as a collector and trainer and for end-game strategy, whatever that may be.
I do notice a drop off of 100 IV mons in recent months since the pokedex number bug fix.
