2500+ Dragonite with best attacking move set.
Stand so far away that couldnt hit it and ran out of balls.
Hatched 3 Eggs simultanously during event, with full pokemon-storage - Incubators used, no PKM, no (doubled) dust, no (doubled) xp
Had a lapras on radar, cycled around hastly, did not find it.
Had a snorlax near home, but he fled after second yellow ball + curved + berry.
Trained a gym to level 10, someone took my place.
That egg thing can't happen. Unhatched eggs are already counted in the amount of pokemon you have. There used to be a bug where sometimes eggs would hatch without showing the hatching animation. With this bug, you would still get the pokemon, candy and stardust. I don't remember when that bug was fixed, but I'd guess that was what actually happened.

93% IV Dragonite with Steel Wing and Dragon Claw and 1985 PC.
I was level 23 and my friend too. He had it with 20 ultra balls but not me. I used something like :
- 50 berry
- 40 ultra balls
- 60 super balls
- 100 poke balls
I touched him at least 1 time every 4 throws, always with curve throws but after half an hour, the dude was still there, laughing at me and my empty inventory ...

Luckily nothing too terrible other than my fat fingers evolving the wrong Charmander to Charmeleon. Rather than doing an 85% one I did the 69% one. Wasted 25 candies which isn't the worst thing. Guess I have to keep collecting.
My worst luck pre-buffs was evolving 2 perfect Eevee's and a 96% all into Jolteons when I wanted a perfect Vap. Still looking for a 80%+ Vap and I'm almost level 31. Sad. I have a ton of 65-70% Eevees I have caught that were about level 29-30 that are now Vaps that I didn't have to invest stardust in and they work great just want that 96%+ one!
Eevees are very common here as I catch them all and I'm sitting at 1500 candies... lol. Why can't I get a good VAP!
Same problem. Only about last week did I realize that the distance you throw the ball doesn't matter at all how fast you swipe, only how FAR/high you swipe.
I've always had difficulty with distant pokemon because I always swiped faster (to throw harder) rather than farther/higher (I guess to throw from a higher angle?) It makes no sense really, but distant pokemon have been a lot easier now.
Also same as some other people here:
-Missed my first Dragonite because my phone signal was too weak in the location the game froze and couldn't reopen (was only around level 7 at the time though, was when there were lots of problems).
-Evolved worst moveset on Arcanine after walking Growlithe for 30 candies during the buddy event
- Had spot stolen in level 10 gym by GPS spoofers (I was only person in the area, trained it the full 8000 prestige)
-Trained a gym from level 8 to 9 alone and then someone took my place. Then I trained from 9 to 10 alone again, and got my place stolen once more. All of this two weeks ago, getting 100~250 prestige per pokemon defeated.
-A Dragonite was at my sightings and I was searching for it (I don't like/never used trackers). Then the Dragonite disappeared on the sightings list, appearing other 9 random pokemon there. I sat down thinking his spawn had already finished. After 5 minutes, a guy appeared saying he had just caught the Dragonite. I ran to it, but at this time his spawn time REALLY was over. I realized in the worst way that Niantic prefer showing Pidgeys and Rattatas on the 9 pokemon that appears on the sightings list instead of showing the important ones.
The used to be a bug when you powered up a pokemon, if your pokemon were sorted by CP and its CP raised above another pokemon then the current displayed pokemon could change to the one you just passed up. When I was powering up an Exeggutor with Confusion+Psychic, it switched to another Exeggutor with Zen Headbutt+Seed Bomb without me realizing it and I ended up wasting probably close to 30k stardust on a pokemon I had been planing to transfer.
There are countless times when screwed by bad evolution movesets or a rare pokemon running away, but actively doing something bad to myself felt so horrible when I was training against the just powered up Exeggutor with the completely wrong moveset and I realized what I had done.
Evolve 5 eggs +90%, none with Solar Beam.
First Gyarados, twister.
Didnt find a lapras because fastpokemap ruined a day before.
Waste 30 candies on wrong exeggutor.
Second dragonite i found, it ran away.
2 Victribels and 2 vileplumes evolved. No solar beam (I think grass type hates me)
I lost a snorlax after sprint 500m from my house.
First porygon i found also ran away.
