Pokédex Glitch?

I scrolled through my pokédex and I suddenly saw an entry for Exeggutor. It said I'd seen one, but I hadn't caught one. However, I never saw one. I also checked my journal, but it also doesn't say anything about Exeggutor. Does anyone have an idea of what could have happened?
UPDATE: Nidoqueen appeared in my pokédex in the same way today. Also, my mom and sister both have the same 5 Pokémon added (Rapidash, Golem, Arcanine, Wigglytuff and Gyarados)

It happened with two friends of mine in the same day.. they saw a dragonite says the pokedex... its a bug

It's not a bug, Niantic has made some changes. Now if you see a new pokemon in a gym that you haven't caught or seen in wild it will appear immediately in your pokedex like "seen".

This is not a bug... it's a new feature, if u saw a new pokemon in gym, u would see it in ur pokedex :)

Now if you see a pokémon on a gym, it will appear on your "seen" ones.

Now if you see a Pokémon in a gym it's going to show up in your pokedex.
