They restrict them to the cities because that way they make sure a big concentration of players go to places where you can find "sponsored" stops. On the note they also pointed to the sponsored Unibail-Rodamco's shopping centers, they want players to visit sponsored places to get a ton of money to Niantic and their partners.
It would be nice that they made them appear anywhere? Yes, of course, it would be nice and fair. Would they make a lot less money from this "event"? Yes too, a ton less, because there will be hundreds of thousands of players travelling to their nearest big city in order to play those days and get their regionals and unowns, this means money, a lot of it. Also with that big amount of players moving they expect us to take the opportunity to farm raids in the city, and this again translates into more money. So we have, money from sponsored malls + money from spinning stops + money from raid passes = a lot of profit to Niantic. They are not a good company at all in game design, game development, customer service, event management, etc. But from a business perspective they know exactly how to maximize their earnings with each move they make.
Of course, Niantic and sponsors aim to make money.
But it's not all about money when they restrict events to big cities. It´s all about logistics. You need to bring people together to make the numbers.
To a place with a good phone signal.
To a place with enough gyms, pokestops and most importantly spawns.
It's a special event. These always will go on in big cities, where they can run smoothly, securely, etc

Unown was captured at lake Bled (Slovenia, Central Europe - Alpine area) already last week. At least reported at local Pokemon GO forums. (info about the location: http://www.burger.si/Bled/uvod_eng.html ), quite a tourist spot also famous by Agata Christie, Paul McCartney, ... maybe good enough for Unown launch...just guessing)
I've already traveled to Copenhagen because of the Safari event that got cancelled later.
i might sound a bit selfish but I think they released these additions to too many cities. They're not rare anymore.
i was hoping they'd appear only in Unibail-Rodamdo owned areas or at least only in the same cities with them but apparently that is not exactly the case (Finland had the capital city, Helsinki, listed while the only Unibail-rodamdo is in Vantaa, a slightly smaller city near Helsinki (not sure if that's the only exception)) so now they become almost-everyone's access, but still not for everyone.
I'd rather have it they appesr roughly in areas tje safari event was supposed to happen, OR just about everyone.
Personally having already traveled across the world to get Kangaskhan and Corsola I feel a bit bummed about this. Sure it's now easier to get every pokemon in my dex but i enjoyed planning my trips to get the rare ones (no, I'm not overly rich elitist believing only the rich should get regionals) but now a lot of people get them for "free" while those who can't travel to the cities they appear in get left out.
Not only that, if it gets narrowed down to Unown and Kangaskhan (which I both have) I feel pretty pissed as I was hoping at least a few new ones. That wording "such as Kangaskhan and Unown" hints at few others as well, not only those two, so there's hoping.
Well... Romania's not even on the list. But hopefully someone will go on vacation to some of those countries and catch a Kangaskan and put it in a gym. It's the only way I'll likely ever get an entry for it, since I doubt I'll ever be able to travel outside of this continent (or out of this country for that matter).
