How does the game generate regular (non-Ex) raids at gyms?
How does the game decide the frequency, time and specific gyms at which raids are generated, as well as the boss or tier for each specific raid? Do the above have constant rates (such as one raid per gym per day with equal chance for each boss); if not, what factors influence them (for instance, gym biome or player activity at the gym)?

All I know is that there cannot be a raid on the same gym in less than 45 minutes since the last one. Also, the most probable time for a raid to take place is right after raid hours have begun, after which the likelihood of raids happening seems to be randomized. Read those two conclusions in TSR long ago but I am too lazy to look for them after so long
The big peak just at the start is very evident for those up early.
However, at least in my town, it seem that there are a secondary peak in the "after work hours".
I am not sure if frequency the raid bosses are completely random. At least last summer Lugia spawns was about 1/3 of the frequency of the other bird at the time. So there is some code implemented to regulate it.
I am a huge solo lvl 3 fan. Earlier this winter I saw some weird pattern for a week or so. One day more than 50% of all lvl 3 spawns were Ninetail, the next day Omastar and the third day Gengar. Maybe a glitch, but extremely unlikely as pure RNG.
I live in a small town with very dense gym areas (and a few outer rim exceptions) and I have never noticed any patterns but I mostly watch out for tier 5s only. There's a place right between downtown and an park from which you can see 11 gyms simultaneously (2 of them EX eligible) and scout for raids and I haven't noticed peaks other than the starting one. Just pure, random chaos all day long. I think I even saw ALL current raid bosses today, as we had only 4 Lugia throughout the day compared to the usual 10-15 legendary raids.
