The worst feeling is when on the slot Machine you drop the quarter and it gets lost. God damn jumping, I can only imagine the Birds will have some sort of unorthodox flight patterns to help me waste balls.

Exactly. One of my two misses was because the excellent throw was headbutted away or jumped over. Slot machine don't typically do that.

Did a quick calculation, given curveball+excellent+golden razz, each hit should have roughly 35% chance, (1-0.35)^11 is less than 1%, about 0.8%, just unlucky..

I usually throw when the Pokemon is about to land from a jump. Sometimes the Pokemon jumps again the moment it lands.

FWIW you are definitely a skilled thrower! I rarely get Excellent! and sometimes not even Great! and I've caught 4 out of 7 Lapras bosses. You have bad luck, though, do not enter the lottery. :-)
Good point about the ice bonus. I do have a +3 ice bonus.

Raid bosses hate excellent, the last time I did a raid battle the boss was a venusaur, I hit him with 3 followed excellent and he fled them all, then I missed miserably and hit him in his foot while he was jumping and then he stayed.

People moaned here coz raid boss was too easy to catch, ''no challenge'' they said. (Ref. early release of raid boss, one ball gotcha! TM is to easy to get etc.). Niantx granted it, now it's too hard to catch. What next?

The point is that the RNG determines the difference between a high-IV, fully-evolved TTar (equivalent to 124 larvitar candies plus having a good latvitar) and nothing at all.
Rewards should be smaller and more cumulative.

I can totally understand the frustration. Last time I got a few excellent throws (all curved + golden razz'd) but that damn Lapras still ran.
